Five ophthalmologist report forms, with section for patient information listing name, residence, age, sex, occupation, diagnosis, referring doctor, O.D.V., O.S.V., and muscle balance. Section for "History, Ophthalmoscopic Examination, &c." includes case notes, illustrations of injured eyes, treatments, and prescriptions. Includes reports for John Banner, an African American porter; John Wissinger, a laborer; Rev. William S. Lowry (b. 1848), an African American minister; Catherine Hart and Mrs. J. Fey, whose occupations were listed as "Wife" and "House" respectively.
